Job Summary

Under general direction, is responsible for entry level professional community health work within the new SA Kids BREATHE (Building Relationships, Effective Asthma Teaching in Home Environments) Program. Individual in this position will provide asthma education and case management services to children and their families in their homes, and work with the child's school nurse and physician. Working conditions are primarily in a community-based environment and an office environment. Exercises no supervision.

The San Antonio Metro Health District (SAMHD) provides public health services to clients, which may include risk for exposure to and possible transmission of vaccine-preventable diseases. Employees in high-risk positions will be required to obtain relevant immunizations, per SAMHD Policy DM13.2 (Download PDF reader). Work Hours
8:00 a.m. - 4:30 p.m. Monday - Friday; flexible hours in early evenings and weekends to meet with working families, as routinely needed; work from home some days.

Essential Job Functions

  • Makes home visits to families of children with asthma, conducts assessments to assist with development of case management plan.
  • Provides asthma education to children and their families using culturally appropriate terms and concepts.
  • Provides consistent, documented follow up of appropriate resource referrals.
  • Conducts data entry to track progress of cases.
  • Participates in case management conferences with team.
  • Follows all rules and laws protecting Protected Health Information (PHI) associated with the Health Insurance Portability and Accountability Act (HIPAA) as well as other regulatory agencies.
  • Employs strategic community building and organizing techniques to engage key internal and community stakeholders around health issues and health equity, including grassroots community based organizations, health centers, schools, community leaders, and community members.
  • Interprets, translates, or provides cultural mediation related to health services or information for community members.
  • Provides feedback to public health providers regarding improving service accessibility or acceptability.
  • Collects information from individuals about the general health of community members.
  • Advocates for individual or community health needs with government agencies or health service providers.
  • Attends community meetings or health fairs to understand community issues and build relationships with community members.
  • Contacts clients in person, by phone, or in writing to ensure they have completed required or recommended actions.
  • Distributes flyers, brochures, or other informational or educational documents to inform members of a targeted community.
  • Performs related duties and fulfill responsibilities as required.

Job Requirements

  • High School Diploma or GED equivalent (recognized by the Texas Education Agency or a regional accrediting agency).
  • Two (2) years of experience working as a Community Health Worker (CHW), health educator, community outreach worker, or community organizer.
  • This position requires a valid Class "C" Texas Driver's License OR a valid driver's license from another state with the ability to obtain a valid Class "C" Texas Driver's License within 30 days of becoming a Texas resident. Use of your own personal vehicle may at times be required and proof of Liability Insurance will be required.
  • Any applicant selected for this position must have a background check completed with results satisfactory to the City of San Antonio and successfully complete the San Antonio's Cash-Handling Certification Training.

  • Unless otherwise stated, applicants are permitted to substitute two years of related full-time experience for one year of higher education or one year of related higher education for two years of experience in order to meet the minimum requirements of the job. One year of full-time experience is defined as 30 or more hours worked per week for 12 months. One year of higher education is defined as 30 credit hours completed at an accredited college or university.
